Disappearing buttons

The small green phone keypad short cut on the home screen has disappeared completely forcing me to reset the whole phone again..

I did momentarily nod off holding the phone..and found a Cnn news app I had downloaded there instead...

Whats happening?

you probably dragged into into the "bin". Press and hold on it from the apps screen too replace it.
