I have been using the free one for quite a while - and it definitely works. I have a custom set up to allow 3g connect at 30 minute intervals when screen is off. It reports on average 1.8 times battery life.
I like the ultimate version for it's location aware wifi control and ability to allow apps to overide settings, for example a dropbox sync app I like to run every 24 hours.
Cool. I used the free version on stock 2.3.4 and found no need for the pro. Since flashing a custom rom I haven't found a need for the app.
I think JuiceDefender will work extra good for me, because I just happen to live in a small area between the good covered wireless data zones. So my phone will sometimes keep searching for a network because the connectivity is so bad here.I've also set my phone to 2G only.
Well, because of this I've given JuiceDefender a chance and bought the Ultimate version.It's also way cheaper than buying spare batteries/emergency chargers, so it's fine even if it doesn't work as expected.

I was averaging 1.7x for battery saving (on notification bar) on stock 2.3.4. But on Darky's I would get 1.1x battery saving, not enough to warrant using it. I get 2.5 days worth of battery power on Darky's 10.1 without any support.
It is a decent app but if I don't use it then I can't recommend paying for it. The free version also gave me enough control.

Is that multiplier thing even close to being accurate? I think of it more as a bragging thing for the app, not really a fact.
Am I wrong?
